If you would like to refer a patient to see a specialist, please have the client call OSVS to make an
appointment. Advise your client as to the service they should be seeing: Internal Medicine, Surgery,
Radiology, Oncology, Exotics, or Ophthalmology. If you are uncertain which service to refer to, please
call and we can advise you. If you do not believe the patient can wait for the next available appointment
with a specialist, you can send the patient through the emergency service and an internal transfer may
be arranged.
Please fax a copy of the medical record (include all recent visits and at least one year history) and any
pertinent laboratory results. If you prefer, you can copy the record and send it with the client. If radiographs
have been taken, please send them with the client (do not mail them unless there are at least 2 weeks before
the appointment for successful delivery).
We will mail the Radiographs back to you promptly.
Please feel free to attach a cover letter if there is information that you feel would be helpful to the case.
A referral form is available if you would find that helpful. After the appointment, you will receive a faxed copy
of the medical record including the diagnostic and treatment plan. Please feel free to call at your convenience
if you would like to discuss the case further with the specialist.
